I was called from work to be lated in at 2300.  So here I am  shiny_smile

Yesterday at work, I got report from the day shift RN about this young patient in his 40's and apparantly he's been drinking and a couple of kids approached him and asked for money.  The patient said, "I don't have any money", and the youngsters proceeded to beat the crop out of him sad smiley.  The patient suffered laceration on his right eyebrow and his right eye swollen shut.  He was hit and kicked by the no good hoodlums and he was brought in by the ambulance.  He was on our floor because his EKG showed atrial fibrillation with rapid response. (his heart rythm is abnormal and his heartrate is fast in the 130's -140's if this continous he could have blood clots and we don't want that in his lungs or in his brain)  Remember the patient is drunk.  So, the plan was he was to be cardioverted or try to reverse his heart rythm to a normal rythm just like most of us are.  Well, the patient had a seizure and fell while in ER but the goodnews is he converted to normal sinus rythm by himself  yes smiley  he stayed in our floor overnight for observation.  No seizures but just pain all over.  His blood alcohol level is 3x above the legal limit.  they did a cat scan of his face and it's negative and a chest xray that showed clear. 

Moral of the story, don't get drunk downtown  whistling

That same night, my second patient was an elderly in his 70's.  He called the ambulance  because he complained of heart bursting out of his chest.  He had multiple cardiac history that's why he got admitted to our floor and that he had a pacemaker as well.  Other than that he had psyche issues.  He was non compliant with his psyche meds and I had to deal with his erratic behavior.  He was writing everything down that's happening.  So, I was doing my assessment and asked him the reason for his pacer and he answered that he doesn't have a heart with a straight face.  lol he wanted to have a cigarette and i told him that he is in a non smoking hospital and that he could not even go outside to smoke because he's on a heart monitor and he has to smoke off the hospital campus.  I told him he's got a nicotine patch on and he replied, "So?"!  ahahahahha.  It's obvious that he was anxious walking up and down the hall thinking the was on some kind of an apartment and one time i caught him talking to his heart monitor so, i decided to give him his ativan with the rest of his night pills. When giving meds we have to ask the patient his name and his birthdate for verification.  So, I asked him his name and his date of birth and he proceeded to ask, "What does the screen say of your computer?"

And I told him, "It doesn't work that way, you have to tell me your name and your date of birth for my verification".  So, he finally relented.  I gave him his pills and he goes on to say, "Are those pills the one that the screen told you to give me?  I told him, "Those pills are the ones that the doctors ordered for you to take".  So, he asked the name of the doctors and the name of the pills that I gave him and wrote it down.  lol He was scheduled for a stress test the next morning.  And guess what?  Sometime the next day, he went AMA.    madsmiley
